If you have a digital signature, then you can fill the entire forms in online. Otherwise you can still fill it in online so its more legable and then print it off and sign it. You then have the option of scanning it and sending it back to us via email, posting or faxing.
*If you get the "this file is corrupted" error then you will need to update your adobe reader program which is completely free.
